III.ii.2.E.2.d. JAG Office | | The derogatory documents (known to VA as facts and circumstances) of all offenses that led to a court martial are kept at the respective Judge Advocate General (JAG) offices. Court martials finalized on or after December 23, 2020, are available online. The Compensation Service Military Court Opinions page directs users to the public databases maintained by each branch of service.If the court martial documents are not available online, send a letter (or e-mail) requesting the copies to the appropriate office and include in the request the - former service member’s

III.ii.2.E.4.c. Facts and Circumstances | | Records containing the facts and circumstances surrounding a service member’s other-than-honorable (OTH) discharge are maintained at the SJA office of the installation from where the service member is discharged. They are typically retained for a period of two years only, after which they are destroyed.Notes: - A request for facts and circumstances must be sent directly to the JAG office.
- Transcripts of a court martial resulting in an OTH discharge may contain facts and circumstances and must also be obtained via written request to the JAG office. Contact information for the JAG office for each service department is located in M21-1, Part III, Subpart ii, 2.E.2.d.

III.ii.2.E.8.a. Address Codes for Service Information | | Service information for Coast Guard personnel who separated from service on or after October 1, 2006, must be requested via DPRIS. For Coast Guard personnel who separated before October 1, 2006, submit the appropriate request to address code 13.Note: The service information discussed in this block refers to- personnel records
- facts and circumstances surrounding an OTH discharge, including
- Board of Officers proceedings, and
- investigations and information concerning all courts martial
- pay grade, and
- travel time.

III.ii.2.E.8.b. Address Codes for STRs and Dental Records | | Beginning September 1, 2014, the Coast Guard stopped sending paper STRs to RMC and began digitizing these records. On June 1, 2015, the Coast Guard began uploading their digitized STRs into HAIMS. The Veterans Benefits Management System (VBMS) automatically generates a Coast Guard STRs request for members with release dates of September 1, 2014, or later. The table below lists the service history, service situations, and the corresponding source to obtain STRs and dental records for officers and enlisted personnel.
