Neurogenic bladder (DC 7542)
Neurogenic bladder is impaired bladder function resulting from injury or disease of the nervous system controlling micturition. Causes include spinal cord injury (most common), multiple sclerosis, stroke, traumatic brain injury, peripheral neuropathy (diabetic, alcoholic), spina bifida, Parkinson disease, and post-pelvic-surgery nerve damage. Depending on the level and pattern of nerve injury, the bladder may be hyperreflexic (overactive, causing urgency, frequency, incontinence) or hyporeflexic (underactive, causing retention, overflow incontinence, recurrent UTIs). The VA does not maintain a separate ladder for neurogenic bladder. Instead, 38 CFR § 4.115a directs raters to evaluate this condition under whichever ladder yields the higher result: voiding dysfunction (0-60%, three sub-formulas) or urinary tract infection (0/10/30%). Apply the ladder matching the predominant symptom.
