Loss of use of both hands (DC 5109)

Body system: Musculoskeletal SystemRegulation: 38 CFR § 4.71a

Diagnostic Code 5109 covers the complete loss of use of both hands, which is one of the most severe musculoskeletal disabilities the VA rates. This doesn't necessarily mean amputation - you could still have your hands physically but be unable to use them functionally due to injury, nerve damage, or other conditions. The VA rates this condition at 100% disability because losing the use of both hands affects virtually every aspect of daily living and work.

Rating levels

  • 100% — You qualify for this rating if you cannot use both of your hands for any practical purpose due to injury, disease, or amputation. This means you're unable to grasp objects, write, type, or perform basic hand functions like buttoning clothes or holding utensils with either hand. The loss of use can be from actual amputation of both hands or from conditions that make both hands completely non-functional.
