Forefoot, amputation proximal to metatarsal bones (DC 5166)

Body system: Musculoskeletal SystemRegulation: 38 CFR § 4.71a

Diagnostic Code 5166 covers amputation of the forefoot where more than half of the metatarsal bones (the long bones in your foot between your ankle and toes) have been lost. This is a fixed amputation rating under the VA's system, meaning the rating percentage is predetermined based on the specific body part affected rather than having multiple rating levels. You'll need to check the full amputation schedule in 38 CFR § 4.71a to find your exact rating percentage, which depends on precisely where and how much of your forefoot was amputated.

Rating levels

  • 40% — You qualify for this rating if you've had an amputation of your forefoot where more than half of your metatarsal bones (the long bones that connect your toes to the middle of your foot) have been removed or lost. The amputation must be close to where these metatarsal bones connect to the middle part of your foot, essentially removing most of the front portion of your foot including where your toes attach.
