Above insertion of pronator teres (DC 5123)

Diagnostic Code 5123 covers amputations or complete loss of use of your arm above where the pronator teres muscle attaches (roughly the middle of your forearm). This is an amputation rating with a fixed percentage based on exactly where your arm was amputated or lost function. The VA uses specific amputation tables in 38 CFR § 4.71a to determine your exact rating percentage, which typically ranges from 60% to 90% depending on the precise location of the amputation.

Rating levels

  • 80% — Amputation of the forearm above the pronator teres insertion (high above-elbow-level forearm loss) is rated 80% on the dominant side and 70% on the non-dominant side per 38 CFR §4.71a.
  • 70% — Amputation of the forearm above the pronator teres insertion (high above-elbow-level forearm loss) is rated 80% on the dominant side and 70% on the non-dominant side per 38 CFR §4.71a.
