Chronic pansinusitis is long-term inflammation involving multiple paranasal sinuses (frontal, ethmoid, maxillary, sphenoid) at the same time. The VA rates this condition under the General Rating Formula for Sinusitis (38 CFR § 4.97) from 0% to 50%, based on episode frequency, treatment intensity (especially prolonged antibiotic courses lasting 4-6 weeks), and post-surgical complications such as chronic osteomyelitis. 50% is the maximum rating under this General Rating Formula.
Rating levels
50% — You qualify for this rating if you've had major sinus surgery that resulted in chronic bone infection (osteomyelitis), or if you have nearly constant sinus problems after multiple surgeries that cause ongoing headaches, pain and tenderness around your sinuses, and thick infected discharge or crusty buildup in your nose and sinuses.
30% — You qualify for this rating if you have chronic sinusitis affecting multiple sinus areas and experience either: three or more severe episodes per year that are so bad they prevent you from normal activities and require 4-6 weeks of antibiotic treatment, OR more than six episodes per year that cause headaches, facial pain, and thick infected discharge or crusty buildup in your nose and sinuses (even if these episodes don't completely disable you).
10% — You qualify for this rating if you have chronic sinusitis (long-term inflammation of your sinuses affecting multiple sinus areas) that causes either 1-2 severe episodes per year that completely knock you out and require 4-6 weeks of antibiotic treatment, or 3-6 less severe episodes per year with symptoms like headaches, sinus pain, and thick infected discharge or dried crusting in your nose. The severe episodes must be bad enough that you can't function normally during that time.
0% — Your chronic sinusitis (long-term inflammation of the sinus cavities) can only be seen on X-rays or other imaging tests, but you don't have any noticeable symptoms that affect your daily life. You might not even realize you have the condition since it's not causing pain, congestion, discharge, or other problems that you can actually feel.
